2010 Registration Form

To register for the 2010 season:

1. Register your student with this online form.

2. (submit by mail.)

3. After downloading, please print and complete the Packet forms, and mail to the address shown on the Packet, along with your check. Your registration will not be complete until this information is received.

Once your complete registration has been received, you will receive confirmation by email that your student's enrollment is complete.

NOTE: If you have questions about which class to register for, or any other questions about these classes, please feel free to contact us any time via email to or via phone at 603-584-1173.

IMPORTANT:

• All classes are filled on a first-come, first-served basis. If we cannot accommodate you when desired, we will do our best to offer a suitable alternative.
• All classes must be paid by check in full at the time of registration.
• The LWSA does not refund or transfer tuition payments, except in the case of doctor's form recommending withdrawal prior to the start of class.
• Your registration will not be complete until the complete Registration Packet, including payment, is received.
• All classes meet regardless of weather.

Don't Forget:

• Please fill out a separate form for each child.
• Fields marked with a * are required for form submission.
• And please remember to click the "Submit" button at the bottom of the form when you're done!
